How Do I Schedule An Appointment?
At Perry Medical Center, we only accept walk-ins, but you can look at our wait times on our home page. When you come in, please bring the following forms to expedite the process and save time. Kids under the age of 18 will need to fill out the Authorization to treat a minor or have a parent/guardian with you.
